We want your visit to be easy!
Your visit with us will be as smooth, and easy as possible. Whether you're looking for a checkup or a renewed prescription, or significant eyecare, we're here to help.
Below are some of the most important items you can bring with you when you visit us. These items help us guide your future and current care and identify your specific needs.
Note: Come in early to request old records to assist in the continuity of your eye health care. This is helpful not only to obtain your current prescription, but to be aware of other medical conditions affecting your eye and/or vision.
What to Bring
Current contact lenses
It is always helpful to bring an old box of the contacts you are currently wearing.
Eye glasses
Bringing in your current eye glasses will allow you to demonstrate to us where they work well and where you have difficulty.
Insurance cards
Insurance cards and your spouse or parent’s information. Many insurance companies have vision insurance, and most give you cards. Please bring those cards with you.
If they are under your significant other’s name, please have his or her social security number and birthday, so we can minimize the delay.
Some insurance carriers do not pay for eye exams, so ask your employer or insurance. You don’t want any surprises.

We would be happy to submit to your insurance for you. However, this doesn’t imply we are providers for all insurance carriers.
It is also important to remember even though you may not have vision insurance, some exams are considered medical and may be covered by your medical insurance. As an optometrist we treat not only the vision but the health of the eye.
We offer a cash discount for paying the same day of exam and not submitting it to insurance. So ask before your exam.
